Regular conversations
Because wishes and needs can change, we like to have regular conversations with every child and family. These are known as advance care planning.
You decide whether you would like to have these conversations with your treatment team or with the Children’s Comfort Team. During these conversations, we look ahead together, and, if possible, with your child as well: what is important to your child? What do you need as a parent? And what does this mean for daily life at home and in the hospital? By regularly reviewing this together, we can continue to provide support that meets your needs.
Based on these conversations, we create an individual care plan for your child. This describes what we do to relieve physical symptoms, how we support psychological and social concerns, which spiritual or existential themes need attention, and how we support your family. The care plan develops along with your child and family and is adjusted when needed. This helps ensure that care continues to fit your situation and wishes as closely as possible.
